RESUMO

Associations between vitamin D deficiency and metabolic syndrome (MS) have been reported; however, the underlying biological mechanisms remain controversial. The aim of this study was to investigate the associations of CYP2R1 and VDR variants with MS and MS components in non-diabetic Brazilian adolescents. This cross-sectional study included 174 adolescents who were classified as overweight/obese. Three CYP2R1 variants and four VDR variants were identified by allelic discrimination. The CYP2R1 polymorphisms, rs12794714 (GG genotype) (odds ratio [OR] = 3.54, 95% confidence interval [CI] = 1.24-10.14, p = 0.023) and rs10741657 (recessive model-GG genotype) (OR = 3.90, 95%CI = 1.18-12.92, p = 0.026) were significantly associated with an increased risk of MS and hyperglycemia, respectively. The AG + GG genotype (dominant model) of the rs2060793 CYP2R1 polymorphism was associated with hyperglycemia protection (OR = 0.28, 95%CI = 0.08-0.92, p = 0.037). Furthermore, the CC genotype (recessive model) of the rs7975232 VDR polymorphism was significantly associated with a risk of hypertension (OR = 5.91, 95%CI = 1.91-18.32, p = 0.002). In conclusion, the CYP2R1 rs12794714 polymorphism could be considered a possible new molecular marker for predicting the risk of MS; CYP2R1 rs10741657 polymorphism and VDR rs7975232 polymorphism are associated with an increased risk of diabetes and hypertension in adolescents with overweight/obesity.

Background: obesity can influence vitamin D levels, which in turn might be associated with cardiometabolic risk factors.Objectives:this study assessed the association between 25-hydroxyvitamin D [25(OH)D] levels and cardiometabolic risk factors in adolescents with overweight living in a region of northeastern Brazil.Material and methods: a cross-sectional study was carried out by non-probabilistic sampling in adolescents diagnosed with overweight or obesity. The subjects were divided according to their 25(OH) D status into two groups: sufficient vitamin D and hypovitaminosis D. Biodemographic, lifestyle, cardiometabolic, and biochemical factors were evaluated. A logistic regression model was applied to determine the predictors of hypovitaminosis D.Results: we found a high frequency of hypovitaminosis D (45.6 %) in adolescents. Weekly sun exposure was negatively associated with hypovitaminosis D (OR = 0.96; 95 % CI: 0.92-0.99), while significant positive associations were observed between hypovitaminosis D and blood pressure above the 95th percentile (OR = 4.00; 95 % CI: 1.19-13.37), body weight (OR = 1.04; 95 % CI: 1.01-1.07), and fasting insulin (OR = 1.13; 95 % CI: 1.05-1.22).Conclusion: hypovitaminosis D showed a high prevalence in adolescents with overweight living in a sunny region of northeastern Brazil, and cardiometabolic risk factors such as systemic arterial hypertension, high body weight, and hyperinsulinemia are predictors of hypovitaminosis D (AU)


Introducción: la obesidad puede influir en los niveles de vitamina D, lo que a su vez podría estar asociado con factores de riesgo cardiometabólico.Objetivos: este estudio evaluó la asociación entre los niveles de 25-hidroxivitamina D [25(OH)D] y los factores de riesgo cardiometabólico en adolescentes con sobrepeso que viven en una región del noreste de Brasil.Material y métodos: se realizó un estudio transversal mediante muestreo no probabilístico con adolescentes diagnosticados de sobrepeso u obesidad. Los sujetos se dividieron según su estado de 25(OH) D en dos grupos: suficiente vitamina D e hipovitaminosis D. Se evaluaron factores biodemográficos, de estilo de vida, cardiometabólicos y bioquímicos. Se aplicó un modelo de regresión logística para determinar los predictores de la hipovitaminosis D.Resultados:encontramos una alta frecuencia de hipovitaminosis D (45,6 %) en los adolescentes. La exposición semanal al sol se asoció negativamente a la hipovitaminosis D (OR = 0,96; IC 95 %: 0,92-0,99), mientras que se observaron asociaciones positivas significativas entre hipovitaminosis D y presión arterial por encima del percentil 95 (OR = 4,00; IC 95 %: 1,19-13,37), peso corporal (OR = 1,04; IC del 95 %: 1,01-1,07) e insulina en ayunas (OR = 1,13; IC del 95 %: 1,05-1,22).Conclusión: la hipovitaminosis D mostró una alta prevalencia entre los adolescentes con sobrepeso que viven en una región soleada del noreste de Brasil, y los factores de riesgo cardiometabólico, como hipertensión arterial sistémica, peso corporal elevado e hiperinsulinemia, son predictores de hipovitaminosis D (AU)

OBJECTIVE: To develop and validate the content of an instrument for assessing the motivation for weight loss in adolescents with overweight and obesity. METHODS: The development and validation of the instrument were conducted in five stages, for which two systematic reviews were conducted. The first one, for the identification of questionnaires assessing the motivation for weight loss, included six studies and contributed to the selection of the domains for the instrument. The second one, conducted to identify the motivations for weight loss in adolescents with overweight and/or obesity, included six studies and contributed to the selection of 17 motivations included in the initial version of the instrument. The motivations most commonly identified were: having better health, improving my appearance, improving my self-esteem and avoiding bullying. The content validity was confirmed by a committee of 12 experts from the areas of nutrition, endocrinology, psychology, and physical education. Based on these evaluations, the content validity index was calculated. Only the items with a content validity index >0.80 for practical relevance were held in the instrument. RESULTS: Five of the 17 motivations included in the initial version of the instrument were excluded because they had content validity index <0.80 for practical relevance. Of the 12 items held in the instrument, five were revised by experts in order to standardize wording and make the language more appropriate for adolescents. Experts categorized the items into the health, personal satisfaction, appearance and social domains. CONCLUSIONS: This is the first instrument assessing the motivation for weight loss in adolescents with overweight and obesity in Brazil. The content validity evaluation by the panel of experts provided more practical relevance, as well as contributed to a better presentation of the items. Further psychometric testing is needed to determine reliability and construct validity of the instrument.

Objective: to verify the association of serum concentrations of 25-hydroxyvitamin D and glycemic levels with the genetic variants rs1544410 and rs2228570 of the VDR gene in adolescents from the Northeast region of Brazil. Materials and methods: a cross-sectional epidemiological study with 208 adolescents from public schools in the city of João Pessoa (Paraíba, Brazil) between 15 and 19 years of age. Blood samples were collected for DNA extraction and analysis of polymorphisms rs1544410 and rs2228570, as well as biochemical analyses (25-hydroxyvitamin D, parathyroid hormone, calcium and glycemia). Results: the mean age was 17.7 (± 1.14) years. Half of adolescents had sufficient serum levels of 25-hydroxyvitamin D and the other half had insufficient/deficient vitamin. The most frequent genotypic distribution was bb and Ff and of lesser frequency BB and ff. There was a significant relationship between the genotypes of rs1544410 and glycemia values (p = 0.049) in the relationships between the genotypes BBxbb (p = 0.012) and Bbxbb (p = 0.037); (p = 0.036, OR = 2.15, 95 % CI = 1.05-4.41), and in the BB+Bb group analysis when compared to the bb (p = 0.025, OR = 1.89, 95 % CI = 1.08-3.29) presented higher risk of glycemia above the median. On the other hand, when Bb+bb was analyzed in relation to BB, adolescents had a greater chance of blood glucose below the median (p = 0.025, OR = 0.66, CI = 0.47-0.95). Conclusion: this study showed a significant relation of glycemia with the distribution of rs1544410 polymorphism genotypes


Objetivo: verificar la asociación de las concentraciones séricas de 25-hidroxivitamina D y los niveles de glucemia con las variantes genéticas rs1544410 y rs2228570 del gen VDR en adolescentes de la región noreste de Brasil. Materiales y métodos: se realizó un estudio epidemiológico transversal con 208 adolescentes de escuelas públicas en la ciudad de João Pessoa (Paraíba, Brasil) de entre 15 y 19 años de edad. Se recogieron muestras de sangre para la extracción de ADN y el análisis de los polimorfismos rs1544410 y rs2228570, así como para análisis bioquímicos (25-hidroxivitamina D, hormona paratiroidea, calcio y glucemia). Resultados: la edad media fue de 17,7 (± 1,14) años. La mitad de los adolescentes tenía niveles séricos suficientes de 25-hidroxivitamina D y la otra mitad, vitamina insuficiente/deficiente. La distribución genotípica más frecuente fue bb y Ff y la de menor frecuencia, BB y ff. Hubo una relación significativa entre los genotipos de rs1544410 y los valores de glucemia (p = 0,049) en las relaciones entre los genotipos BBxbb (p = 0,012) y Bbxbb (p = 0,037); (p = 0,036, OR = 2,15, IC 95 % = 1,05-4.41), y el análisis del grupo BB + Bb en comparación con el bb (p = 0,025, OR = 1,89, IC 95 % = 1,08-3,29) mostró un mayor el riesgo de glucemia, por encima de la mediana. Por otro lado, cuando se analizó Bb+bb en relación con la BB, los adolescentes tuvieron una mayor probabilidad de que la glucosa en sangre estuviera por debajo de la mediana (p = 0,025, OR = 0,66, IC = 0,47-0,95). INTRODUCTION: Objective: this study aimed to investigate the association between serum 25-hydroxyvitamin D (25 [OH] D) and cardiometabolic risk factors in Brazilian adolescents. Methods: a cross-sectional study with 220 school adolescents aged 15 to 19 years was performed in the city of João Pessoa-PB, Brazil. The variables studied were: 25 hydroxyvitamin D (25 OH), dietary intake of vitamin D, anthropometric data (body mass index (BMI), waist circumference (WC) and waist / height ratio (RCA) and biochemistry, total cholesterol (TC), Low Density Lipoprotein (LDL), High Density Lipoproteins (HDL), triglyceride (TG), C-reactive protein (CRP), alpha acid glycoprotein (AGPA), malondialdehyde (MDA) and total antioxidant capacity (CAT), blood pressure, parathyroid hormone (PTH) and serum calcium. Enough and insufficient /deficient adolescents were compared by qui-square test and analysis of multiple lineal regression was accomplished to identify the factors cardiometabólicos associated to the seric concentration of 25 (OH) D (p < 0,05). Results: a total of 57.3% had vitamin D insufficiency/deficiency, which was more prevalent in females (79.35%). The chi-square test revealed an evident relationship between the three indicators involved in overweight assessed in this study BMI (p = 0.01), WC (p = 0.04) and AGR (p = 0.02). There was an association between the independent variable (vitamin D) and calcium and triglyceride levels. Conclusion: a high proportion of adolescents presenting insufficiency/vitamin D deficiency was ssociated with cardiometabolic risk factors.


INTRODUCCIÓN: Objetivo: este estudio tuvo como objetivo investigar la asociación entre la 25-hidroxivitamina D sérica (25 [OH] D) y los factores de riesgo cardiometabólico en adolescentes brasileños. Metodologia: se realizó un estudio transversal con 220 adolescentes escolares de 15 a 19 años en la ciudad de João Pessoa-PB, Brasil. Las variables estudiadas fueron: 25 hidroxivitamina D (25 OH), ingesta dietética de vitamina D, datos antropométricos (índice de masa corporal (IMC), circunferencia de la cintura (WC) y relación cintura / altura (RCA) y bioquímica, colesterol total (TC) , Lipoproteínas de baja densidad (LDL), lipoproteínas de alta densidad (HDL), triglicéridos (TG), proteína C reactiva (PCR), glicoproteína alfaácida (AGPA), malondialdehído (MDA) y capacidad antioxidante total (CAT), presión arterial, Hormona paratiroidea (PTH) y calcio sérico. Se compararon adolescentes suficientes e insuficientes/deficientes mediante la prueba de qui cuadrado y se realizó un análisis de regresión lineal múltiple para identificar los factores cardiometabólicos asociados a la concentración sérica de 25 (OH) D (p < 0,05). Resultados: un 57,3% tenía insuficiencia/deficiencia de vitamina D, que era más prevalente en las mujeres (79,35%). La prueba de ji cuadrado reveló una relación evidente entre los tres indicadores involucrados en el sobrepeso evaluados en este estudio IMC (p = 0,01), WC (p = 0,04) y AGR (p = 0,02). Hubo una asociación entre la variable independiente (vitamina D) y los niveles de calcio y triglicéridos. Conclusión: una alta proporción de adolescentes con insuficiencia de deficiencia de vitamina D de 25 (OH) D se asociaron con factores de riesgo cardiometabólico. Conclusión: una alta proporción de adolescentes con insuficiencia/deficiencia de vitamina D se asoció con factores de riesgo cardiometabólico.

There was an association between the independent variable (vitamin D) and calcium and triglyceride levels. Conclusion: a high proportion of adolescents presenting insufficiency/vitamin D deficiency was ssociated with cardiometabolic risk factors


Objetivo: este estudio tuvo como objetivo investigar la asociación entre la 25-hidroxivitamina D sérica (25 [OH] D) y los factores de riesgo cardiometabólico en adolescentes brasileños. Metodologia: se realizó un estudio transversal con 220 adolescentes escolares de 15 a 19 años en la ciudad de João Pessoa-PB, Brasil. Las variables estudiadas fueron: 25 hidroxivitamina D (25 OH), ingesta dietética de vitamina D, datos antropométricos (índice de masa corporal (IMC), circunferencia de la cintura (WC) y relación cintura / altura (RCA) y bioquímica, colesterol total (TC) , Lipoproteínas de baja densidad (LDL), lipoproteínas de alta densidad (HDL), triglicéridos (TG), proteína C reactiva (PCR), glicoproteína alfaácida (AGPA), malondialdehído (MDA) y capacidad antioxidante total (CAT), presión arterial, Hormona paratiroidea (PTH) y calcio sérico. Se compararon adolescentes suficientes e insuficientes / deficientes mediante la prueba de qui cuadrado y se realizó un análisis de regresión lineal múltiple para identificar los factores cardiometabólicos asociados a la concentración sérica de 25 (OH) D (p < 0,05). Resultados: un 57,3% tenía insuficiencia / deficiencia de vitamina D, que era más prevalente en las mujeres (79,35%). La prueba de ji cuadrado reveló una relación evidente entre los tres indicadores involucrados en el sobrepeso evaluados en este estudio IMC (p = 0,01), WC (p = 0,04) y AGR (p = 0,02). Hubo una asociación entre la variable independiente (vitamina D) y los niveles de calcio y triglicéridos. Conclusión: una alta proporción de adolescentes con insufi ciencia de deficiencia de vitamina D de 25 (OH) D se asociaron con factores de riesgo cardiometabólico. Background: Hypovitaminosis D is a frequent problem in the world and can be influenced by several factors. Objective: To estimate the prevalence of hypovitaminosis D and associated factors in adolescent students of a capital city of northeastern Brazil. Methods: Cross-sectional epidemiological study assessing 220 school adolescents of both sexes aged 15 to 19 years. A questionnaire was applied to assess sociodemographic (skin color, mother’s education and receiving social benefits) and behavioral data (number of hours of sleep per day, sun exposure, physical activity and dietary vitamin D intake). Anthropometric nutritional status was assessed by BMI (body mass index) for age. Serum 25 (OH) D concentrations, PTH and serum calcium were measured by blood samples. We identified individuals with hypovitaminosis D as those with 25 (OH) D < 30 ng/dL. The analysis of factors associated with hypovitaminosis D was performed by simple and multivariate Poisson Regression. Results: The prevalence of hypovitaminosis D was 57.3%. The female population had a significantly higher prevalence than the male. In the final adjusted model, the variable independently associated with hypovitaminosis D in females was the lowest serum calcium concentrations, and for males it was BMI, in which adolescents who had overweight/obesity were 2.4 times more likely to have hypovitaminosis D than the eutrophic ones. Conclusions: The present study found a high prevalence of hypovitaminosis D in the analyzed population, especially in female adolescents. In addition, the factors independently associated with hypovitaminosis D were found to be overweight/obese in boys and low calcium concentrations in girls (AU)


Introducción: la hipovitaminosis D es un problema frecuente en el mundo y puede verse influenciada por varios factores. Objetivo: estimar la prevalencia de hipovitaminosis D y factores asociados en adolescentes de una ciudad capital del noreste de Brasil. Métodos: estudio epidemiológico transversal que evaluó a 220 adolescentes de ambos sexos de 15 a 19 años de edad. Se aplicó un cuestionario para evaluar los datos sociodemográficos y de comportamiento. El estado nutricional antropométrico fue evaluado por IMC para la edad. Las concentraciones séricas de 25 (OH) D, PTH y calcio en suero se midieron mediante muestras de sangre. Se identificaron individuos con hipovitaminosis D como aquellos con 25 (OH) D < 30 ng/dl. El análisis de factores asociados con la hipovitaminosis D se realizó mediante Regresión de Poisson simple y multivariante. Resultados: la prevalencia de hipovitaminosis D fue del 57,3%. La población femenina tenía una prevalencia significativamente mayor que la masculina. En el modelo ajustado final, la variable independientemente asociada con la hipovitaminosis D en las mujeres fue la más baja de las concentraciones séricas de calcio, y para los hombres fue el IMC, en el que los adolescentes con sobrepeso/obesidad tenían 2,4 veces más probabilidades de tener hipovitaminosis que los eutróficos. Conclusiones: el presente estudio encontró una alta prevalencia de hipovitaminosis D en la población analizada, especialmente en adolescentes. Además, se encontró que los factores asociados de forma independiente con la hipovitaminosis D fueron el sobrepeso/obesidad en los niños y concentraciones bajas de calcio en las niñas (AU)

ABSTRACT Objective: To investigate the association between serum level of 25-hydroxy vitamin D and the Vitamin D Receptos gene BsmI polymorphism in the blood profile of community-dwelling older adults. Methods: This cross-sectional study included 142 older males and females. A questionnaire collected socio demographic information, medical history, and factors associated with sun exposure. Weight, height, and waist circumference were measured. Biological material was collected to analyze biochemical parameters 25-hydroxy vitamin D, parathormone, serum calcium, urea, creatinine, liver enzymes, and blood profile) and to verify the presence of the vitamin D receptos gene BsmI polymorphism. Results: Most participants were female (80.3%). The mean levels of 25-hydroxy vitamin D, hemoglobin, and hematocrit were 32.1±7.3 ng/dL, 13.5±1.5 d/dL, and 40.0±4.4%, respectively. Fifty-eight (40.8%) participants had vitamin D insufficiency/deficiency (25.7±3.3 ng/mL), and 18 (12.6%) had anemia. Serum vitamin D was associated with hemoglobin (p=0.030) and hematocrit (p=0.032). However, when subjects were categorized as anemic or not anemic, said association was not maintained (p=0.270). Moreover, the BsmI polymorphism was not associated with hemoglobin and hematocrit levels, regardless of vitamin D status. Conclusion: The serum level of vitamin D is associated with hematocrit and hemoglobin levels in older adults. However, these blood parameters were not associated with the vitamin D receptor gene BsmI polymorphism.


RESUMO Objetivo: Investigar a associação do perfil hematológico com os níveis séricos de 25-hidroxivitamina D e polimorfismo BsmI do gene vitamina D receptor de idosos não institucionalizados. Métodos: Estudo transversal realizado com 142 idosos de ambos os sexos. Foram coletadas, por meio de um questionário, informações sociodemográficas, antecedentes clínicos e fatores associados à exposição solar. Além disso, foram aferidas medidas antropométricas de peso, altura e circunferência da cintura. Material biológico foi coletado para análise dos parâmetros bioquímicos (25-hidroxivitamina D, parathormone, cálcio sérico, ureia, creatinina, enzimas hepáticas e perfil hematológico) e para identificação do polimorfismo BsmI do gene vitamina D receptor. Resultados: Os participantes do estudo eram, em sua maioria, do sexo feminino (80,3%) e apresentaram concentrações médias de 25-hidroxivitamina D, hemoglobina e hematócrito de 32,1±7,3 ng/dL, 13,5±1,5 d/dL e 40,0±4,4%, respectivamente. Da amostra total, 40,8% (n=58) apresentaram insuficiência/deficiência de vitamina D (25,7±3,3 ng/mL) e 12,6% (n=18), anemia. Encontrou-se associação entre as concentrações séricas de vitamina D com as de hemoglobina (p=0,030) e hematócrito (p=0,032). Entretanto, quando os sujeitos foram categorizados quanto à presença ou não de anemia, essa associação não se manteve (p=0,270). Além disso, não foi observada relação entre o polimorfismo BsmI e as concentrações de hemoglobina e hematócrito nos grupos de idosos com suficiência ou insuficiência/deficiência de vitamina D. Conclusão: O presente estudo encontrou associação entre as concentrações séricas de vitamina D e as de hematócrito e hemoglobina nos idosos analisados. Contudo, não foram observadas associações entre esses parâmetros hematológicos e o polimorfismo BsmI do gene vitamina D receptor.

RESUMO

A experiência em destaque apresenta como objetivos articular comunidade e serviço de saúde na construção de ações para o desenvolvimento social no domínio comunitário e promover a formação acadêmica dos estudantes envolvidos, por meio de ações de ensino e pesquisa no âmbito da extensão universitária. O Projeto Práticas Integrais da Nutrição na Atenção Básica em Saúde (PINAB) atua, desde 2007, em ações de fortalecimento da formação crítica dos sujeitos da comunidade e dos serviços, qualificando as bases para a promoção da Gestão Estratégica e Participativa no território, além da criação de espaços de encontro e diálogo comunitário, protagonizados por educadores populares e pelo incremento de dimensões críticas, compreensivas e participativas nas ações de saúde na comunidade, na Unidade de Saúde da Família e na Escola local.


The highlighted experience has the goal to articulate community and health services in the construction of actions for social development in the community domain and promote academic training of students involved through teaching and research action within the university extension. The Integral Practices on Nutrition in Primary Health Care Project (PINAB) operates since 2007 in critical actions to strengthen the training of individuals from the community and services, improving the foundation for the promotion of participatory management in the territory and the creation of spaces and community dialogue played by popular educators and the increase of critical dimensions, comprehensive and participatory in health care in the community, Family health Unit and the local school.
